Vaping is not "just water vapour"
A common misconception is that e-cigarettes produce harmless water vapour. In reality, they heat a liquid — usually a mix of propylene glycol, vegetable glycerine, flavourings and (in most products) nicotine — into an aerosol of fine particles that the user inhales deep into the lungs.
That aerosol is not chemically neutral. Depending on the device and liquid, it can contain volatile organic compounds, carbonyl compounds such as formaldehyde produced when the liquid is heated, ultrafine particles, and traces of metals from the heating coil. Inhaling these substances repeatedly, over months and years, is fundamentally different from breathing clean air.
Effects on the lungs and airways
The lungs are the organ most directly exposed to vaping. Reported and studied effects include:
- Airway irritation and inflammation. Many users report coughing, throat irritation, wheezing and increased mucus, especially when they begin vaping.
- Reduced lung function in some studies. Research has associated vaping with measurable short-term changes in how the lungs work, though scientists are still mapping the long-term picture.
- Serious lung injury in rare cases. A cluster of severe lung-injury cases (often referred to as EVALI) was strongly linked to certain additives, particularly in illicit or modified products. It demonstrated that inhaling the wrong substances can cause acute, dangerous harm.
Shortness of breath, chest pain, a persistent cough, fever or vomiting in someone who vapes should be assessed by a medical professional without delay.
Effects on the heart and circulation
Nicotine is a stimulant. When absorbed, it raises heart rate and blood pressure and causes blood vessels to narrow. Over time, repeated exposure places extra strain on the cardiovascular system. Researchers are actively studying how habitual vaping affects long-term heart and blood-vessel health, and early evidence suggests it is far from risk-free.
Nicotine and the brain
Most e-cigarettes deliver nicotine — often in high concentrations and in smooth "salt" formulations that make large doses easy to inhale. Nicotine is powerfully addictive. In adolescents and young adults, whose brains are still developing into their mid-twenties, exposure can affect attention, learning, mood and impulse control, and can prime the brain for dependence.
Because nicotine reaches the brain within seconds, the cycle of craving and relief can establish itself remarkably quickly — sometimes within weeks of regular use.
The long-term unknowns
Cigarettes took decades of study before their full harms were understood. E-cigarettes are comparatively new, and the devices and liquids keep changing. That means we do not yet have the long-term data that would tell us the full consequences of vaping over 20, 30 or 40 years. "We have not proven it is dangerous over decades" is very different from "we have proven it is safe."
